Specialized Habilitation (DD/ SLS waiver)-

Directly working with adults on daily living skills within a safe environment that promotes comprehensive services such as independent living skills, budgeting, meal planning, and developing social skills with fellow participants.

 

Supported Community Connector (DD/ SLS waiver)-

Directly working with adults on social skills, daily living skills, volunteer opportunities, and making meaningful connections with community participants. These are community-based services that support participants ability to access the community in a safe environment while engaging in personalized goals that address specific needs while in the community.
